What Happens Next
Your application will be reviewed by our Referral Coordinators. Applications are reviewed in the order that we receive them.
You will get an email from Hope Diagnostic Center within 48 hours with your intake paperwork.
- Once all paperwork (application and intake) is completed, appointments will be scheduled.

Helpful Information for Your Child’s Upcoming Evaluation
To make your future appointment smoother, you may find it beneficial to start gathering records and information.
Items to bring with you
Notes about your child’s development and behaviors (what you’ve observed)
Any previous records (doctor visits, therapy reports, school/early intervention evaluations)
Comfort items your child likes (toy, snack, iPad, etc.)
Any communication tools your child uses
- A list of questions or concerns you have
What to expect
During your child’s evaluation, the diagnostician will work to understand how your child learns, interacts, and communicates.
